Specialized staff, including registered nurses knowledgeable in post-acute care, provide medical and rehabilitative support, oversee clinical care and are focused on returning patients back to their lifestyle. Our skilled nursing centers must meet specific criteria to earn the MedBridge name.
We offer a variety of long-term residential options featuring varying levels of care and independence. We also have a memory care unit at our Heartland of
Centerville facility. Residents often come from the hospital, home or other community residences and need daily assistance. Our center is designed to provide gracious daily living and personal care in a homelike environment.
