Brookhaven is a continuing care community including independent cottages and apartments, assisted living apartments and skilled nursing and rehab for an age in place concept. Brookhaven is located in a rural small town on a beautiful campus with walking space, ponds, trees, and gorgeous sunrise and sunset views. The true small town with a family kind of feeling amongst the campus. There is opportunity to age in place. Pets are allowed in Independent Living. There is an onsite restaurant and meal plans available for independent living. Rehab offers PT, OT, ST services in a separate wing of the nursing home. The nursing facility can accommodate many complex clinical needs. Secure Memory Care Unit is offered in both the skilled nursing and assisted living. The campus is owned by share holders in the community, managed by HCF Management.
